About our speaker:

David J. Forman, LISW-S, is a social worker with extensive clinical and forensic professional experience in both public and private sectors. 

Forman is the owner and CEO of Assessments & Interventions, a business that provides evidence-based drug and alcohol assessments and treatment, as well as forensic evaluations and consultative services, to governmental and non-governmental entities.

Since 2004, he has also held an administrative position with the State of Ohio Department of Mental Health & Addiction Services.

As the forensic admissions and legal assurance director for Twin Valley Behavioral Healthcare, he manages the legal assurance department for a 276-bed behavioral health inpatient psychiatric hospital, supervising forensic social workers and providing counsel to hospital staff and community stakeholders in all 88 Ohio counties.

Additionally, he has chaired committees and statewide task forces to promote the effective delivery of forensic psychiatric services.

He holds a master’s degree in clinical social work from the University of Louisville, a bachelor’s degree from Eastern Kentucky University, and an associate degree from Golden West Community College .
