 820 High St. Speaker: David Forman\, LISW-S\, Ohio Department of Mental He
 alth & Addiction Services Learn more about the impact of mental illness an
 d the resources available to help families\, businesses and communities in
  Franklin County. About our speaker: David J. Forman\, LISW-S\, is a socia
 l worker with extensive clinical and forensic professional experience in b
 oth public and private sectors. Forman is the owner and CEO of Assessments
  & Interventions\, a business that provides evidence-based drug and alcoho
 l assessments and treatment\, as well as forensic evaluations and consulta
 tive services\, to governmental and non-governmental entities. Since 2004\
 , he has also held an administrative position with the State of Ohio Depar
 tment of Mental Health & Addiction Services. As the forensic admissions an
 d legal assurance director for Twin Valley Behavioral Healthcare\, he mana
 ges the legal assurance department for a 276-bed behavioral health inpatie
 nt psychiatric hospital\, supervising forensic social workers and providin
 g counsel to hospital staff and community stakeholders in all 88 Ohio coun
 ties. Additionally\, he has chaired committees and statewide task forces t
 o promote the effective delivery of forensic psychiatric services. He hold
 s a master’s degree in clinical social work from the University of Louis
 ville\, a bachelor’s degree from Eastern Kentucky University\, and an as
